Amazingly special effects without much effort! Although this is my first test, it already looks quite good I think. I had to try over a few times, because you have to be really careful and precise on what gets blurred and what not.

Here you can see more results of this wonderful technique. These effects are reached by manipulating on normal photograph with image editing software.

The real tilt-shift photography involves special camera movements and a large aperture to get a very limited depth of field. You can read all about it on Wikipedia.
